Richard W. (Dick) Shivley, 65, Burnettsville, died Saturday, Dec. 1, 2012, at Indiana University Health, White Memorial Hospital Emergency Room in Monticello.
Services will be at 2 p.m. Tuesday at the Miller-Roscka Funeral Home of Monticello, with the Rev. Paul Scott of the Burnettsville First Baptist Church of Burnettsville, officiating.
Burial will be in the Davis Cemetery located in rural Burnettsville. Visitation will be at 11 a.m. until the time of service Tuesday.
Memorials may be given to the American Heart Association.
